Short answer

Healthcare software vendors worth shortlisting in 2026 split into large consultancies (Cognizant, Thoughtworks), healthcare specialists (ScienceSoft, Topflight Apps, Orangesoft, Belitsoft, Vention) and offshore or nearshore teams (10Pearls, Unosquare, Innovation Insight). Only ScienceSoft and Belitsoft state ISO 13485 on the pages we checked. Innovation Insight publishes its prices: a telehealth MVP at $35k to $60k and a regulated platform at $80k to $200k+. Ask every vendor to sign a BAA before any PHI moves.

Disclosure: we are one of the companies listed; we wrote this list and applied the same criteria to ourselves.

How we built this list

This list covers companies that build custom healthcare software for clients: telehealth, patient apps, EHR integrations and medical device software. It does not cover EHR product vendors. We opened each company's own healthcare pages in October 2026. Compliance wording matters a great deal in this sector, so we report it exactly as each company states it. "HIPAA compliant" is not a certification anyone can hold; ISO 13485 and ISO 27001 are. The criteria:

  • Facts come only from the company's own website, opened in October 2026. If a page did not state a fact (a founding year, a headquarters), we left it out rather than filling it from directories or memory.
  • Each company had to show at least one named, public project or client in this area. Logo walls without a described project did not count.
  • No scores and no ranking. Companies are grouped by type, and the "best for" line reflects what each one publicly shows, not a private judgement of quality.
  • Prices are listed only where the company publishes them. Most do not publish prices.
  • Innovation Insight is held to the same rules: only work from our own published case studies, and only the prices on our pricing page.

Large consultancies

Cognizant

Cognizant's healthcare technology pages centre on payer and provider platforms: claims auto-adjudication, TriZetto Facets, revenue cycle management and electronic prior authorisation, with FHIR mentioned for the latter. Named case studies include claims automation for El Paso Health and a revenue cycle programme for MetroHealth. Much of this is platform work rather than greenfield product builds. Best for: health plans and large provider groups modernising claims and billing.

Thoughtworks

Thoughtworks, founded in Chicago in 1993, presents its healthcare and life sciences work around legacy R&D systems, AI-ready data products and clinical trial analytics. Named clients include Bayer, for AI on preclinical data, and Seagen, a Pfizer company. Its pages do not name specific certifications. Best for: pharma and medtech groups that need data platforms and engineering practice change more than an app.

Healthcare specialists

ScienceSoft

ScienceSoft lists McKinney, Texas, says it has worked in healthcare IT since 2005, and states it is certified under ISO 13485, ISO 9001 and ISO 27001 with HL7 FHIR certified implementers on staff. It describes HIPAA experience rather than a HIPAA certification, which is the accurate way to say it. Named projects include an app for the University of New Mexico Health Sciences Center and a lung cancer detection application for bioAffinity Technologies. Best for: software as a medical device and other work that needs an ISO 13485 quality system.

Topflight Apps

Topflight Apps is in Irvine, California, and builds HIPAA-compliant web and mobile apps, AI agents and integrations using HL7, FHIR and SMART on FHIR, with BAAs and audit logging. Its portfolio names GaleAI, a medical coding product, and its homepage carries a testimonial from a Stanford Medicine physician. It makes no ISO claims. Best for: US digital-health startups that need Epic or FHIR integration in their first release.

Orangesoft

Orangesoft describes itself as a custom healthcare software company founded in 2011, building medical device software, telemedicine, EHR integrations and mental health apps. Its footer says HIPAA and GDPR compliant and ISO certified, with ISO 9001 and HL7 FHIR logos. Its named projects are described without client names: a stroke telemedicine app and a virtual hospital system. Best for: an EU-based team for patient-facing and telemedicine apps.

Belitsoft

Belitsoft lists offices in Alexandria, Virginia, London and Warsaw, and states ISO 13485:2016 certification, with experience across HL7, ICD-10, CPT, HIPAA, HITECH and FDA requirements. Its named project is modernising a portable ultrasound app for E.I. Medical Imaging; other cases, including an FDA-cleared stethoscope app, are anonymised. Best for: SaMD or EHR work on an Eastern European budget.

Vention

Vention lists a New York office and covers EHR, telemedicine, medical device software, digital therapeutics and wearables, stating HIPAA, GDPR and HITECH compliance and an ISO 27001 certification. Named health clients include Dialogue and Thirty Madison. Best for: funded health-tech startups that need to add engineers within weeks.

Offshore and nearshore teams

10Pearls

10Pearls delivers from Pakistan and several Latin American countries. Its healthcare page treats HIPAA, GDPR, HL7/FHIR and ISO 27001 as design constraints and says it is a certified Epic App Orchard developer. Named portfolio projects include an advance care planning app and a musculoskeletal telemedicine platform. Best for: US companies that want a US-managed team with offshore cost.

Unosquare

Unosquare was founded in 2009 and runs offices in the US, Mexico, Colombia, Bolivia and Belfast. Its healthcare work spans diagnostics, remote monitoring, payer platforms and EHR systems, and its named case studies include embedded engineering for athenahealth. Best for: US healthcare companies that want embedded engineers in their own time zone.

Innovation Insight

Innovation Insight was founded in 2019, is headquartered in Lahore with an engineering office in Sialkot, and has 50+ engineers, designers and project managers. Our healthcare work: for Zeuss, a US telehealth provider, we built a patient portal, a CRM for doctors and agents, and an event-driven back end of 37+ Azure Functions, with PHI encrypted at rest, audit logging of every state-changing request, Spreedly for PCI-compliant payment tokenisation and pharmacy fulfilment integration. For CareFlair we built an Australian NDIS marketplace with WCAG-aligned design. We do not hold ISO 13485 and have not built FDA-regulated device software, so for SaMD the specialists above are the better fit. Published prices: a patient app MVP at $30k to $60k, a telehealth MVP at $35k to $60k, EHR integration at $15k to $40k. See our HIPAA-compliant software development page.

Who we checked and left out

Itransition, Emorphis Health, Mobisoft Infotech and Chetu passed most checks and were left out for length or because their portfolio projects named no clients. EPAM, Globant, BairesDev and Kanda Software could not be reached when we checked. Mindbowser's healthcare page loaded but named no clients.

How to choose a healthcare software development company

  1. Ask the vendor to sign a business associate agreement before any protected health information reaches them, and to name every subprocessor in the data path.
  2. Separate certifications from claims. ISO 13485 and ISO 27001 come with a certificate number and an auditor; ask for both. "HIPAA compliant" describes how a system is built and run, not the vendor.
  3. If your product could be a medical device, ask about IEC 62304 and design controls before scoping, not after.
  4. Ask which EHRs they have integrated with, through which API (FHIR R4, HL7 v2, vendor APIs), and whether it went through Epic or Oracle Health app review.
  5. Ask how test data is handled. Developers should never need real patient records to do their work.
  6. Ask for two references from projects like yours, and call them. A case study on a website is the vendor's version of events.
  7. Interview the engineers who will actually do the work, not only the sales lead or a solutions architect who leaves after the kickoff.
  8. Get the rate card by seniority and the planned team mix in writing. A low blended rate often hides a junior-heavy team.

FAQ

Related questions.

Can a software company be HIPAA certified?

No. There is no official HIPAA certification. A vendor can sign a business associate agreement, follow the Security Rule's safeguards and hold related certifications such as ISO 27001 or SOC 2, which an independent auditor checks.

How much does a healthcare software development company charge?

Most do not publish prices. Innovation Insight publishes $35k to $60k for a telehealth MVP and $80k to $200k+ for a full regulated platform.

Do I need a vendor with ISO 13485?

Only if your software is, or may become, a medical device. For telehealth, scheduling and patient engagement apps, HIPAA safeguards, a BAA and good security practice matter more.

Is it safe to outsource healthcare software offshore?

It can be, with a BAA, access only through your own cloud accounts, no PHI on developer machines, and audit logs. Several vendors on this list deliver from Pakistan, Latin America or Eastern Europe for US healthcare clients.

Which vendors on this list have Epic experience?

10Pearls states it is a certified Epic App Orchard developer, and Topflight Apps lists SMART on FHIR integration. Ask any vendor for the specific EHR integrations they have shipped.
