Short answer

A healthcare app costs roughly $30k to $60k for a patient-facing MVP, $80k to $200k or more for a telehealth or practice management platform, and $15k to $40k for each EHR integration, based on Innovation Insight's published rates of $25 to $49 an hour. HIPAA safeguards add about 15 to 25 percent over a comparable non-regulated app. Budget another 15 to 20 percent of the build cost each year for maintenance, plus hosting, video and integration fees.

How much does a healthcare app cost?

App typeTypical scopeBuild costTimeline
Wellness or symptom tracker (no PHI for a provider)Consumer app, content, tracking, reminders$20k to $45k8 to 12 weeks
Patient portal or app MVPAppointments, messages, documents, payments, one platform plus web$30k to $60k10 to 16 weeks
Telemedicine platformVideo visits, scheduling, intake, e-prescribing, web, iOS and Android$80k to $200k+5 to 9 months
Remote patient monitoringDevice data ingestion, alerts, clinician dashboards, billing codes$60k to $150k4 to 8 months
Practice management or clinic SaaSMulti-tenant scheduling, records, billing, reporting$80k to $200k+5 to 10 months
EHR or FHIR integration (per system)Read and write patient data with Epic, Oracle Health or others$15k to $40k4 to 10 weeks

These ranges come from our own proposals and delivered projects at published rates of $25 to $49 an hour for senior engineers. A US-based agency charging $100 to $200 an hour would typically quote two to four times more for the same scope. See pricing for the rate card.

What HIPAA adds to the budget

HIPAA compliance adds roughly 15 to 25 percent to a comparable non-regulated app. The extra work is concrete: audit logging on every access to patient data, role-based access and multi-factor authentication, encryption and key management, stripping PHI from notifications and analytics, a documented risk analysis, a penetration test and the policies to go with them. Our guide on how to make an app HIPAA compliant lists each safeguard.

Compliance itemTypical effortNotes
Audit logging and access reports1 to 2 weeksAppend-only log with a reviewable UI
Access control, MFA, session rules1 to 2 weeksSSO with the client's identity provider adds time
Encryption, key management, backups3 to 5 daysMostly configuration on HIPAA-eligible cloud services
Risk analysis and documentation1 weekUpdated whenever the architecture changes
Penetration test$4k to $12kExternal firm; retest after fixes

The biggest cost drivers

  • Integrations. Each EHR, pharmacy, lab, insurance or device integration is its own mini-project, and vendor approval processes can add weeks.
  • Number of user roles. Patients, clinicians, admins, billing staff and caregivers each need their own flows and permissions.
  • Platforms. Web plus iOS plus Android costs less with a cross-platform framework such as Flutter or React Native, but still more than one platform.
  • Video and real-time features. Video visits, chat and live dashboards need careful engineering for weak connections.
  • Clinical workflow complexity. Intake rules, triage logic, care plans and billing codes are where scope grows quietly.
  • Accessibility. Older patients and assistive technology users need WCAG-level design and testing from the start.

Running costs to budget for

CostWhat drives itTypical range
Cloud hosting on HIPAA-eligible servicesTraffic, data volume, redundancy$300 to $3,000 a month for most startups
Video minutesNumber and length of visitsUsage-based, from the video provider under a BAA
E-prescribing, identity, fax and SMS servicesPrescribers and message volumePer-prescriber or per-message fees
Monitoring, logging, backupsRetention and volume$100 to $800 a month
Maintenance and updatesOS releases, security patches, small fixes15 to 20 percent of build cost a year, or a retainer from $1,500 a month
Get every vendor's BAA and pricing before you commit to an architecture. Some services charge more for their HIPAA tier, and a few will not sign a BAA at all.

Three ways to lower the cost without cutting corners

  1. Launch one workflow first. A telehealth MVP with booking, video, notes and payments proves demand before e-prescribing and insurance billing.
  2. Buy regulated components. Use a BAA-covered video SDK, identity provider and e-prescribing service instead of building them.
  3. Share one codebase. Flutter or React Native for patient apps, with a web app for clinicians, keeps one team on the whole product.

Healthcare app cost by feature

If you already know your feature list, this table is a quicker way to sanity-check a quote. Each line assumes a HIPAA-ready implementation by a senior team and includes design, development and testing. Add the lines for your app, then add 15 to 20 percent for project management, security testing and launch.

FeatureApproximate costNotes
Patient sign-up, identity verification and consent$3k to $7kID checks and e-signature through third-party services
Appointment booking and reminders$4k to $9kMore for multi-clinic, multi-time-zone scheduling
Secure messaging and file sharing$4k to $8kEncrypted storage, audit trail, attachment scanning
Video visits$6k to $15kBAA-covered SDK, waiting room, reconnect handling
Clinician notes and care plans$5k to $12kTemplates, structured fields, sign-off
E-prescribing$6k to $14kIntegration with an e-prescribing network provider
Payments and memberships$3k to $7kStripe for cash-pay; insurance adds more
Insurance eligibility and claims data$8k to $20kClearinghouse APIs, superbills
Admin dashboard and reporting$5k to $12kUtilisation, revenue and audit reports
Wearable or device data (RPM)$8k to $25kPer device family; alerting rules extra

Offshore, onshore or in-house: how the choice changes the number

The same healthcare app can cost very different amounts depending on who builds it. The difference is mostly the hourly rate; the compliance work is the same wherever the engineers sit, and a vendor outside the US signs the same business associate agreement and follows the same rules.

OptionSenior hourly ratePatient app MVP (about 800 to 1,200 hours)Trade-offs
US agency$100 to $200$80k to $240kSame time zone; highest cost
Western European agency$85 to $170$70k to $200kStrong GDPR experience; high cost
Eastern European agency$45 to $90$40k to $110kGood overlap with Europe
Pakistan (Innovation Insight)$25 to $49$30k to $60kMorning overlap with the US East Coast, full overlap with Europe
In-house US teamSalary, benefits and recruiting$250k+ per year for a small teamFull control; slow to hire; ongoing cost

Our software development rates by country guide explains how these ranges were compiled and what to check before comparing quotes.

A worked example

A US startup wants a virtual clinic for dermatology: patients book, upload photos, have a video visit and pay; clinicians see a queue, write notes and send prescriptions. Discovery and design take two weeks ($6k). The MVP, with a Flutter patient app, a Next.js clinician web app, Stripe payments, Twilio video under a BAA and an e-prescribing integration, takes about 14 weeks with a team of two senior engineers, a designer part-time, QA and a delivery lead: roughly $55k to $70k. A penetration test adds about $6k. Hosting and services start around $600 a month before video usage.
